Policies
In order to provide reliable, evidence based medical and psychiatric mental health services while protecting the safety of our patients, Center State Wellness, LLC limits its scope of practice to specialty services offered by appointment only. All medical, psychiatric, or behavioral health services / advice will be given only during a scheduled office visit, occurring in person or via tele-health.
Emergency Services
We do not offer “on-call” or emergency services. In the event you have a medical emergency, or psychiatric mental health crisis, or you are feeling suicidal, homicidal, or having thoughts of self harm, call 911, report to the nearest emergency department, or contact one of the following agencies:

No Show / Cancellations
Recognizing that everyone’s time is valuable and appointment times are limited, we ask that you arrive on time for your appointments, and provide a 48 (forty eight) hour notice if you are unable to keep your appointment. In the event that you miss your appointment, or do not provide 48 hours notice, you will be charged an administrative fee. The fee is expected to be paid and a follow up appointment scheduled prior to any medication refills.
Refills
We request that you provide a two business day (48 hour) notice for any refill.
The only acceptable method to request a refill is by phone. Electronic pharmacy generated refill requests will not be honored.
If your pharmacy requests a prior-authorization please inform this office immediately, as this will increase the time required to process your refill. Medications lost or stolen will not be refilled until the next scheduled refill date. Medications will not be refilled after hours, on weekends, or major holidays.
Inclement Weather / Closings
If you have a scheduled appointment during adverse weather conditions, please call ahead to verify if the office will be open. If the office is closed due to inclement weather, your visit will commence as scheduled via tele-health.
